Checkbox 5 Question Items Guide

Checkbox Survey Items are categorized by Question Items, Display Items, Report Items, and Action Items. This guide focuses on Questions Items and provides detailed descriptions of the 10 question types below. Each question type has a unique Item Editor. This guide describes the tools and options available in each Item Editor.

Question Item Types

The Question Items menu is the default menu when you select the "Add Item" button within the Survey Editor. This menu consists of:

  • 5 Multiple choice items: Radio Button, Drop Down List, Checkboxes, Rating Scale, Slider
  • 2 Open response items: Open-Ended Single-Line Text, Open-Ended Multi-Line Text
  • 1 Matrix item: A grid of multiple questions
  • 1 CAPTCHA item: Display a CAPTCHA input that must validate for the survey to proceed

CHECKBOX TIP: Select an item type to preview the item on the right side of the window.

Radio Buttons

Radio Button questions present a list of possible answer options from which only one option can be selected.

The Radio Button editor consists of six editor tabs and one preview tab

  • Preview: Displays the current question set-up. The preview updates after changes are saved
  • Question Text: Main question text, also known as the question label. Consistent among question item types
  • Description: Subtext to the main question text. Sits below the question text line. Consistent among question item types
  • Choices: Possible answer options are entered here. The Choices tab is not applicable to all question items
  • Behavior: Question settings. Behavior options vary among question item types
  • Appearance: Question label and choice appearance settings. Appearance options vary among question item types
  • Conditions: Set item conditions. Consistent among question item types

Question Text:

Main question text, also known as the question label.

  1. Enter question text in the HTML or TEXT view
  2. Use the WYSIWYG editor to format text in HTML view. WIthin this editor you can insert images, create tables, change font characteristics, create links, and more
  3. Merge in text from previous questions, User Attributes, or Response Properties

This tab contains optional behavior settings that you can choose to apply to your survey:

  1. Create a question alias. An alias acts as alternate question text and can be applied when reporting or exporting response data
  2. Check "Answer is required" to make this a required question. This will place an asterisk (*) to the left of your question text. Respondents will not be able to proceed in the survey until an answer is selected
  3. Check "Randomize Options" to randomly reorder the answer options for this question every time the survey is opened. Randomizing answer options can help prevent answer bias
  4. Check "Allow Other" to allow respondents to enter their own answer instead of selecting one of the answers provided. Enter default text into the "Other" text field to prompt the respondent. You can also apply merging here

CHECKBOX TIP: Reports and exported data display the question text from your survey editor. This means that if you used merging in your question text, the source value code that was applied to your question will appear in your report or export. Report viewers may be confused as to what the question is asking because of the source value code. This is an example of when you might want to use an alias, or alternate text, in place of your actual question text.

Appearance:

Make changes to the appearance of questions and answers here.

  1. Change the layout of answer choices. Choose between Vertical to Horizontal
  2. Arrange answer choices into columns to avoid scrolling. Especially handy when you have a long list of answer options
  3. Show number labels to the left of answer choices
  4. Change the justification of the entire question in relation to the survey content frame. Options include left justified, right justified, or centered in the window
  5. Change the position of the question text in relation to the answer choices. Options include top, bottom, left, or right

Conditions:

Use conditional logic on survey items to ensure that a particular question will only display when certain conditions are met by the respondent. If no item conditions are set, the survey item will always* display in your survey.

* Even if no item conditions are set, if one or more page conditions are set, the entire page may or may not display depending on whether certain conditions are met by the respondent. Items are subject to the conditions (if any) set to their page.

Drop-Down List

Like Radio Button questions, Drop-Down List questions allow a respondent to make only one answer selection. Drop-Down Lists are useful for hiding long list of answer options.

The Drop-Down List editor consists of six editor tabs and one preview tab:

  • Preview: Displays the current question set-up. The preview updates after changes are saved
  • Question Text: Main question text, also known as the question label. Consistent among question item types
  • Description: Subtext to the main question text. Sits below the question text line. Consistent among question item types
  • Choices: Possible answer options are entered here. The Choices tab is not applicable to all question items
  • Behavior: Question settings. Behavior options vary among question item types

Checkboxes

Unlike Radio Button and Drop-Down List questions, Checkbox questions allow a respondent to select multiple answer options from the list of answer choices. You can specify the minimum and maximum number of answers a respondent is allowed to select under the Behavior tab.

  2. Enter an number greater than "0" in the Min. field to make this a required question. This will place an asterisk (*) to the left of your question text. Respondents will not be able to proceed in the survey until they have selected the appropriate number of answer choices. For example, by entering "1" in the Min. field, you are requiring respondents to select at least one answer in order to continue
  3. Enter a number in the Max. field if you wish to limit the number of answer choices a respondent
  4. Check "Randomize Options" to randomly reorder the answer options for this question every time the survey is opened. Randomizing answer options can help prevent answer bias
  5. Check "Allow Other" to allow respondents to enter their own answer instead of selecting one of the answers provided. Enter default text into the "Other" text field to prompt the respondent. You can also apply merging here

Rating Scale

A Rating Scale question consists of a consecutively numbered scale, the range of which can be customized under the Behavior tab. Rating Scale questions allow a respondent to choose only one answer choice from the scale range.

NOTE: When generating a report, only Rating Scale questions can be displayed as Statistics Table charts.

  1. Create a question alias. An alias acts as alternate question text and can be applied when reporting or exporting response data
  2. The number entered in the Scale Start Value field will be the first number in your rating scale. The scale will increase in numerical order until the End Scale Value is reached. A radio button will sit above the each number in the scale. There is no limit to the size of your scale
  3. Scale Start Text sits above the first number in the scale. Scale Mid Text sits above the middle number(s) of the scale. Scale End Text sits above the last number in your scale. Make sure the start, middle and end text are assigned to the correct end of the scale. For example, if you display a scale from 1-5 and 1 is equal to very dissatisfied, make sure "Very Dissatisfied" is entered in Scale Start Text, not Scale End Text. Scale text is optional
  4. Check "Answer is required" to make this a required question. This will place an asterisk (*) to the left of your question text. Respondents will not be able to proceed in the survey until an answer is selected
  5. Checking "Enabel N/A Option" will place an additional radio button to the right of the rating scale. Customize the N/A text in the field provided. You can also apply merging here

  5. Set the width between scale increments in pixels. For example, if you enter 100px in the text field provided, each scale option will be 100px apart

Matrix

Matrix items are made up of rows and columns to create a grid of questions. Every row/coloumn combination is a unique question.

NOTE: When generating a report, Matrix Items can only be displayed as summary tables.

The Matrix question editor consists of six editor tabs and one preview tab:

  • Preview: Displays the current question set-up. The preview updates after changes are saved
  • Question Text: Main question text, also known as the question label. Consistent among question item types
  • Description: Subtext to the main question text. Sits below the question text line. Consistent among question item types
  • Rows: Each row contains a subject variable.
  • Columns: Each column contains a question variable.
  • Appearance: Matrix label and grid appearance settings. Appearance options vary among question item types
  • Conditions: Set item conditions. Consistent among question item types

NOTE: Row conditions can be applied on the Preview tab using the blue arrows to the left of the row labels (1). Setting a condition to a row means that that row will either display or not display to the respondent depending on whether certain conditions are met.

Rows:

Each row represents the subject of a question asked in the matrix column(s). Similar to the Choices tab in other question editors, the Rows tab lets you enter variables line by line.

  1. Select the orange '+' icon to add a row. Select the red '-' icon to remove a row. Select in the row to edit answer text
  2. Rearrange rows by selecting the corresponding up/down arrow and dragging the variable to a different position
  3. Select the blue number labels to the left of the row label to edit the text
  4. Use merging to insert the answer to a previous question, User Attribute, or Response Property into the answer choice field

Columns:

Each column asks a question directed toward the matrix row variable(s), or subject(s).

  1. The first column in the table is populated automatically as the "Row Texts" column. This column is not editable
  2. In order to add a question to the matrix you must create a new column. Select the Add Column button to add a new column

After selecting the Add Column button, a window will appear with question type options. In this example, we will make our first column a Rating Scale question.

Select Next to set column properties.

On the Properties step you can enter column question text, and configure question behavior options and column options. In this example we will not include any question text.

On the Properties step under Behavior, configure your question item. The behavior options will depend on the question type you selected on the Select Column Type step.

On the Properties step under Column Options, configure your column settings.

  1. Determine whether the answer to each question in this column should be unique. This only applies when the matrix consist of more than one row and the number of rows is less than or equal to the number of answer choices in the column
  2. Set the width of the column in pixels

Select Finish complete the column setup process.

On the Complete step of the column setup wizard you have the option to add more items/columns to your matrix, or to exit the wizard and return to the Matrix Item Editor. In this example we will exit the wizard.

As you can see, the new Rating Scale column has been added to the column list. To modify this column, select the blue number to the left of the column type label (1).

  1. Choose to add grid lines to the matrix table. Options include vertical lines, horizontal lines, or both
  2. Set the width for the entire matrix in pixels. If you do not set column widths, they will automatically adjust to fit the matrix width set here
  3. Change the justification of the entire question in relation to the survey content frame. Options include left justified, right justified, or centered in the window
  4. Change the position of the question text in relation to the answer choices. Options include top, bottom, left, or right

Open-Ended Single-Line Text

Open-Ended Single-Line Text items are used for collecting a single line of free form text. Formatting rules can be applied to the answer text. This type of question is useful for collecting demographic data and contact information.

The Open-Ended Single-Line Text item editor consists of five editor tabs and one preview tab:

  • Preview: Displays the current question set-up. The preview updates after changes are saved
  • Question Text: Main question text, also known as the question label. Consistent among question item types
  • Description: Subtext to the main question text. Sits below the question text line. Consistent among question item types
  • Behavior: Question settings. Behavior options vary among question item types
  • Appearance: Question label and choice appearance settings. Appearance options vary among question item types
  • Conditions: Set item conditions. Consistent among question item types

Specify an answer format, such as Email Address, Date, All Numbers, All Letters, etc. Checkbox will validate respondents' answers based on whether or not they entered an answer in the format specified here. If a respondent enters an invalid format, they will see a validation alert after attempting to proceed in the survey
  2. Check "Answer is required" to make this a required question. This will place an asterisk (*) to the left of your question text. Respondents will not be able to proceed in the survey until an answer is selected
  3. Create a question alias. An alias acts as alternate question text and can be applied when reporting or exporting response data
  4. Enter Default Text here to display pre-populated text in the question text field when a respondent enters the survey page. This text can be deleted or modified by the respondent. You may user merging here
  5. Limit the amount of text a respondent can enter in this Open-Ended Text item by entering a character number into the Max. Length field

CHECKBOX TIP: To minimize scrolling on contact or registration forms, position the text field on the same line as the question text by setting the Label Position to 'Left' or 'Right'

Open-Ended Multi-Line Text

Open-Ended Multi-Line Text items are used for collecting multiple lines of free form text within a specified number of rows and columns. This type of question is useful for collecting open response answers, such as explanations, suggestions, comments, questions, etc.

The Open-Ended Multi-Line Text item editor consists of five editor tabs and one preview tab:

  • Preview: Displays the current question set-up. The preview updates after changes are saved
  • Question Text: Main question text, also known as the question label. Consistent among question item types
  • Description: Subtext to the main question text. Sits below the question text line. Consistent among question item types
  • Behavior: Question settings. Behavior options vary among question item types
  • Appearance: Question label and choice appearance settings. Appearance options vary among question item types
  • Conditions: Set item conditions. Consistent among question item types

  2. Specify the number of Columns included in the text box. This will determine how wide the box is.

Conditions:

File Upload

Add a File Upload input option to allow respondents to upload a file form a computer. The file will be captured with the survey response.

NOTE: The default maximum file size that can be uploaded is 4MB. Customers with server licenses can set the maximum file size to their specifications.

Behavior:

This tab contains optional behavior settings that you can choose to apply to your survey:

  1. Check "Answer is required" to make this a required question. This will place an asterisk (*) to the left of your question text. Respondents will not be able to proceed in the survey until an answer is selected
  2. Create a question alias. An alias acts as alternate question text and can be applied when reporting or exporting response data
  3. Select which file type(s) respondents are permitted to upload to their response. The file type(s) you select here will be noted on the item in your survey.

CHECKBOX TIP: Reports and exported data display the question text from your survey editor. This means that if you used merging in your question text, the source value code that was applied to your question will appear in your report or export. Report viewers may be confused as to what the question is asking because of the source value code. This is an example of when you might want to use an alias, or alternate text, in place of your actual question text.

Captcha

Include a Captcha validation input to protect your survey from internet bots. Captcha displays a unique code that the respondent must enter into the text field provided. If the code entered is incorrect, the respondent will not be able to proceed in the survey.

  1. Create a question alias. An alias acts as alternate question text and can be applied when reporting or exporting response data
  2. Specify the Minimum Code Length of the Captcha item
  3. Specify the Maximum Code Length of the Captcha item
  4. Select a Captcha Code Type. Options include Alpha, Numeric, and AlphaNumeric.
  5. Set the size of the Captcha code Image Width
  6. Set the size of the Captcha code Image Height
  7. Determine the Image Format

Text Formats:

Select one or more Captcha styles from the Text Formats list. If you select more than one format, the styles will rotate.

Appearance:

Change the justification of the entire question in relation to the survey content frame. Options include left justified, right justified, or centered in the window.

Slider

The Slider Item allows respondents to drag a key along a horizontal plane and drop it at a desired value along the path. Slider answer values can be configured as a numeric range, or with custom answer choices.

NOTE: The Slider question type is unique among its peers in that a default answer value will always be selected. Slider questions cannot be left blank.

Behavior:

The Value Type is selected on the Behavior tab in the item editor. Choose from:

  • Number Range (numeric scale)
  • Image (upload images to use as answer choices)
  • Text (text answer choices).

When Number Range is selected, additional options will appear below the value type drop-down menu.

  1. Minimum Value = Scale start value. Appears on the left side of the scale.
  2. Maximum Value = Scale end value. Appears on the right side of the scale.
  3. Step Size = The number of values/points between scale markers. Entering ’1′ will increase the numeric scale by one point at a time as you drag the key across the scale from left to right (i.e. 0, 1, 2, 3….). Entering ’2′ will increase the numeric scale by two points at a time as you drag the key across the scale (0, 2, 4, 6…).
  4. Default Value = Determines where the key will be located when a respondent reaches the question. By default, the key will be positioned at the Minimum Value on the far left of the scale. If your Slider scale rage is 0-100, setting a Default Value of 50 will place the key in the center of the scale.

When Image is selected, no additional options will appear on the Behavior tab. Images are uploaded on the Choices tab.

When Text is selected, no additional options will appear on the Behavior tab. Answer values are entered on the Choices tab.

Choices:

Possible answer values are entered/uploaded here.

NOTE: When Number Range is selected, the Choices tab is disabled.

When Image is selected on the Behavior tab, the images are uploaded on the Choices tab.

Select the orange Upload button to upload your first item.

A window will appear. To add an image, choose from the following options:

  • Upload New Image (select file to upload)
  • Use Image From Web Site (enter image URL)
  • Use Previously Uploaded Image (select from drop-down list)

After adding an image, select Set The Image.

Next, create an alias for your image. A unique alias is required for all options when using images. The alias value for images will be shown when viewing reports or exporting data.

Select the "+" icon to add your image.

NOTE: You may upload up to 10 images per slider item.

When Text is selected on the Behavior tab, the custom values are entered on the Choices tab.

Enter answer choices line by line in the Normal View:

    1.    Select the orange '+' icon to add a line. Select the red '-' icon to remove a line. Select in the row to edit answer text
    2.    Rearrange answer choices by selecting the corresponding up/down arrow and dragging the answer choice to a different position
    3.    Select the Default radio button to the left of an answer choice to have that option pre-selected in your survey
    5.    Create aliases for answer choices. In the Choices tab, an alias is alternate answer text, which can be applied when reporting and exporting response data
    
CHECKBOX TIP: Include a “N/A” value if your Slider question doesn’t apply to all respondents.

Cut and paste answer choices from a text file into Quick Entry View:

Appearance:

Make changes to the appearance of questions and answers here.

When Number Range is selected as the slider type:

  1. Set the Slider width in pixels.
  2. Checking the Show Value box will display the selected numeric value on the far left of the scale. As you move the Slider key from left to right, you will see the this value change.

When Image is selected as the slider type:

  1. Set the Slider width in pixels.
  2. Set the image alias to display above images, below images, or to not show at all.
  3. Set the image position to display above or below the slider bar.

When Text is selected as the slider type:

  1. Set the Slider width in pixels
